COVID-19 Self-Screening: Before Your Appointment at CCH

Ahead of attending an appointment at Cornwall Community Hospital (CCH), it’s important to confirm that you are in good health and are not experiencing symptoms of COVID-19. Working together, we can help prevent spread of the virus.

To serve you better, CCH has developed a COVID-19 self-screening tool that will help us process your arrival at CCH more smoothly. Below, you’ll find details sharing:

1. Who should fill out the COVID-19 self-screening tool

Patients who are coming to the hospital for the following reasons should fill out the self-screening tool:
  • To attend a clinic
  • For a diagnostic imaging appointment
  • To have a procedure
  • If you are undergoing surgery
Please note this tool is NOT meant for those with virtual or telephone appointments.

2. How to fill out the COVID-19 self-screening tool

There are two ways to fill out the COVID-19 self-screening tool:
  1. Upon arrival at CCH
  2. – or –

  3. Up to 24-hours before arrival at CCH

1. Self-screening upon arrival at CCH:

Patients can fill out the self-screening tool upon arrival at CCH. Tablets will be provided at the front entrance of the hospital (at the screener’s desk) so you can fill out the self-screening tool on-site. Remember to have your health card with you.

 

2. Self-screening before arrival at CCH:

Starting February 8, 2021, when you receive a scheduled appointment from CCH, you will be asked for permission to obtain your e-mail address. The hospital can then send appointment details and the self-screening tool e-form to you electronically.

This e-form will walk you through a series of COVID-19 symptoms and questions developed to show whether you meet screening requirements before entering the building.


Remember to:

  • Prepare ahead: Self-screening from home should be filled out up to 24 hours before arrival at CCH.
  • Print/Take a screen shot: Choose “print screen” on your mobile device, computer or tablet, or take a screen shot and bring your self-screening results with you to CCH.
  • Show: Be prepared to show your health card and your self-screening results to the screener at CCH’s door so they may verify your results.

3. Frequently Asked Questions

Q. What do I do with my self-screening results at CCH (pass or fail)?

If you pass your self-screening, take your results to the screener’s desk. Present your health card as well as your results to the screener for verification.

Q. What should I do if I have trouble filling out the self-screening tool?

If you have any trouble completing the COVID-19 self-screening tool, please go to our screening desk upon arrival at the hospital and a member of our staff will be happy to assist you.

 

Q. How do I prepare for arrival at CCH?

  • Please wear a mask to the hospital. If you do not have one, one will be provided to you.
  • Upon your arrival, please show the door screener your self-screening confirmation and your Health Card. You may still be asked some limited questions and will be asked to sanitize your hands.
  • Please practice physical distancing and keep two meters from others as much as possible.
  • If you have any questions or concerns in advance of your arrival, please contact your care provider or clinic directly.
