Heart Function Clinic

The Heart Function Clinic at CCH is a specialized outpatient cardiac clinic for patients with heart failure, a condition that develops when your heart doesn't pump enough blood for your body's needs. This can happen if your heart can't fill up with enough blood or when your heart is too weak to pump properly. The Heart Function Clinic is part of Cornwall Hospital’s Chronic Disease Management Program, located on the third floor of the hospital. The Heart Function Clinic at CCH provides specialized care for patients living with Heart Failure, including appointments with a multi-disciplinary team, such as a cardiologist (heart doctor) and nurse practitioner, who help monitor and manage heart failure symptoms, provide coaching and support, and develop individualized treatment plans to promote better heart health, quality of life and reduce hospitalizations.
